General Information

Thank you for visiting our office for your consultation and evaluation. We are pleased to let you know that most insurance companies and Medicare may cover surgery for your leg veins. Surgery means the endovenous laser ablation operations which we perform to eliminate an underlying cause for most vein disorders. Insurance may also pay for the microphlebectomy procedure, removing some of the larger veins from your legs through a very small one millimeter micropuncture. Insurance may also pay for injection of some of the larger veins in order to “dry up” these veins and remove them from circulation. The only treatment which will not be covered by insurance is the cosmetic spider vein treatment.

Most managed health care plans, (HMOs), require a referral authorization. It is the patient’s responsibility to obtain such authorization and have the referral faxed to our office, or bring the referral form to your appointment. By arranging for the referral, you will be able to avoid having to reschedule the appointment for a later date.

Many insurance plans have a fixed deductible for the year or require a copay or co-insurance payment at time of service. Please verify your patient responsibility with your insurance company prior to your visit, as this amount will be collected at your visit.

We are a participating provider with Medicare; therefore, we accept the amount Medicare allows as payment. The patient is responsible for the 20% difference between the Medicare allowable and payment. This will be collected at your visit, unless you have a supplemental insurance plan which we will then bill separately. Most supplemental policies will pay the 20% that Medicare does not pay. Please verify any patient responsibility amount with your supplemental insurance provider.

Conservative therapy before surgery is often required by insurance companies and by Medicare. This means that you should wear medical grade compression hosiery, elevate your legs, and walk or otherwise exercise almost every day. You should perhaps keep a record of wearing the hosiery so that we can use this record should we have to send information to the insurance company. If you have not worn the stockings, please purchase a pair during your initial visit with us and wear them as much as you can. These stockings are very good for your legs.
